Hazard Log

Overview

The "Hazard Log" shall contain the recorded information of the safety justification, or Safety case, and shall provide a summary of all safety activities throughout the system life cycle.
The "Hazard Log" shall track the progress on the resolution of risks associated with the identified hazards i.e., those contained in the hazard list.
The Hazard Log shall contain details of each hazard and accident, and shall be updated as new hazards are discovered, or if there are changes to identified hazard or accident data.

Details

The "Hazard Log" shall be established at the start of the project and maintained to provide a record of the progress on resolving risks associated with the identified hazards. The procedure shall be within or referenced from the "System Safety Program Plan".
The Hazard Log shall be prepared for the applicable System and Subsystems, together with the safety requirements.
The "Hazard Log" shall reference all analyses and reports produced during the system safety program plan and shall include:

• severity categories
• probability categories
• equivalent numerical probabilities
• accident risk classification scheme
• design rules and techniques
• partitioning of random and systematic elements of the hazard probability targets between system functions

Each "Hazard Log" shall contain the following information for each identified accident:

• unique identification
• a summary
• accident severity category and probability targets appropriate to risk classes B and C
• A cross reference to the full description and analysis of the accident sequence

The 'Hazard Log' shall contain the following data for each hazard:

• a unique identification (for reference purposes)
• a summary of the hazard including the functions and CIs and their states that represent the hazard, together with a reference to the appropriate design documentation.
