Hazard Log

Overview

The "Hazard Log" shall contain the recorded information of the safety justification, or Safety case, and shall provide a summary of all safety activities throughout the system life cycle.
The "Hazard Log" shall track the progress on the resolution of risks associated with the identified hazards i.e., those contained in the hazard list.
The Hazard Log shall contain details of each hazard and accident, and shall be updated as new hazards are discovered, or if there are changes to identified hazard or accident data.

Details

The "Hazard Log" shall be established at the start of the project and maintained to provide a record of the progress on resolving risks associated with the identified hazards. The procedure shall be within or referenced from the "System Safety Program Plan".
The Hazard Log shall be prepared for the applicable System and Subsystems, together with the safety requirements.
The "Hazard Log" shall reference all analyses and reports produced during the system safety program plan and shall include:
