The precise format of this report should be followed.
Overview
The 'Problem/change report' shall
be used to report Class II problems and changes (deficiencies)
to the developmental baselines of configuration items (CIs) and
to the design documents and source code listings entered into
the internal developmental configuration for a CSCI or HWCI.
NOTE: Class I changes to established baselines shall be
reported via formal methods defined in the 'Configuration Management
Plan'.
