Preliminary Hazard Analysis
Overview
Major hazards associated with the system which has
an effect on safety shall be analyzed. These hazards shall be
controlled by engineering or eliminated by re-design.
Preliminary Hazard Analysis shall be performed in the form of
a study or similar method agreed with the acquirer (purchaser).
Hazards not previously identified by the 'Preliminary Hazard Listing'
shall be entered into the 'Hazard Log'
and the entries for any existing hazards and accident sequences
updated.
The 'Preliminary Hazard Analysis Report' shall be referenced by
the 'Hazard Log'.
The content 'Preliminary Hazard Analysis Report'
shall be as follows:
- - a brief description of the system and its design;
- - a brief description of any subsystems identified;
- - a list of identified hazards applicable to
the system including a description and unique reference;
- - a list of identified accidents applicable to
the system including a description, a unique reference and a description
of associated hazards and accident sequence;
- - the accident risk classification scheme adopted;
- - Preliminary probability targets for each accident;
- - preliminary predicted probabilities for each
accident sequence;
- - preliminary probability targets for each hazard;
- - a description of the system functions and safety
features;
- - a description of human error which could create
or contribute to accidents;
- - list of all source documents used, including
their issue, dates, and status;
- - conclusion as and recommendations.
