DOCUMENTARY REQUIREMENT FOR CIVIL REGISTRY CERTIFICATION

Applicants for Philippine civil registry certifications (birth, death, marriage, etc.) from the National Statistics Office (NSO), Manila are informed of the following:

A.     Data requirements for:

1. BIRTH AND CERTIFICATE OF NO MARRIAGE RECORDS

  • Complete name
  • Date of birth
  • Place of birth
  • Names of parents
  • 2. MARRIAGE

  • Complete names of contracting parties

  • Date of marriage

  • Place of marriage

  • 3. DEATH

  • Complete name of the deceased

  • Date of death

  • Place of death

  • 4. LEGAL INSTRUMENTS (acknowledgment, legitimation, etc.)

  • Type of legal instrument (subject matter)

  • Date and place where it was subscribed and sworn

  • 5. COURT DECREES (adoption, annulment of marriage, etc.)

  • Name of the court that rendered the decision

  • Case number and title of the case

  • Date when the decision was rendered

    B.   The certification fee is U.S.$15.00 to be remitted by the requesting party thru the Philippine Veterans Bank, Corner Herrera and Dela Rosa Streets, Legaspi Village, Makati City, Philippines; tel. nos (632)893-57-66 and (632) 857-83-67; fax no (632) 840-36-63.  The National Statistics Office (NSO) Dollar Account Number is 0016-00098-6. The requesting party should enclose a proof of remittance (bank receipts, etc.), otherwise, the request for certification will be withheld pending receipt of notice from the Philippine Veterans Bank.  NSO’s address is Civil Registry Division, Vibal Bldg., Times St. corner EDSA, West Triangle, Quezon City; tel. no. 00(632) 926-73-73; fax no. 00(632) 926-73-29.
     

    The NSO can not immediately respond to foreign requests for civil registry certifications if the information given by the requesting party is incomplete.
