Applicants for Philippine civil registry
certifications (birth, death, marriage, etc.) from the National Statistics
Office (NSO), Manila are informed of the following:

B. The certification fee
is U.S.$15.00 to be remitted by the requesting party thru the Philippine
Veterans Bank, Corner Herrera and Dela Rosa Streets, Legaspi Village, Makati
City, Philippines; tel. nos (632)893-57-66 and (632) 857-83-67; fax no (632)
840-36-63. The National Statistics Office (NSO) Dollar Account Number is
0016-00098-6. The requesting party should enclose a proof of remittance (bank
receipts, etc.), otherwise, the request for certification will be withheld
pending receipt of notice from the Philippine Veterans Bank. NSO’s
address is Civil Registry Division, Vibal Bldg., Times St. corner EDSA, West
Triangle, Quezon City; tel. no. 00(632) 926-73-73; fax no. 00(632) 926-73-29.
The NSO can not immediately respond
to foreign requests for civil registry certifications if the information given
by the requesting party is incomplete.
