APPLICATION FOR WAIVER

(FBCCC NO. 1 FORM)


REQUIREMENTS

  1. Set of FBCCC No. 1 form, duly accomplished and signed by the exporter and shipper or their authorized representatives;

  2. Copy each of the following documents as enclosures:

    3. a. Certificate of Non-Availability of Philippine-flag vessel obtained from 

    SUARDIAZ SHIPPING LTD.

    b. If goods are to be shipped by air:
     

    • sales invoice

    • bill of lading or airway bill, whichever is applicable

    • letter of credit

  3. Certification fee is £36.00 in British postal money order or company cheque

    4. * Please send a self-addressed stamped envelope if the document will be returned by post.
