Fee is £36.00 plus £18.00 for registration of birth
- Original and three (3) clear
photocopies of Birth Certificate ( Showing both parent’s names )

Available at the Embassy )
- Three (3) passport size photos with
white background (see FORMS)
- Three (3) photocopies of parent’s
valid Philippine passport or Naturalisation paper if former Filipino

PASSPORT NEEDS AMENDMENT
A. IF MARRIAGE WAS
SOLEMNIZED ABROAD
- Valid passport
- Completed Amendment Form ( If applying
for amendment only, fee is £15.00 plus £18.00 for
registration of marriage, no pictures needed )
- Completed Passport Application Form (
If applying for renewal with amendment of name, fee is £36.00 plus £18.00
for registration of marriage)
- Original and three (3) clear
photocopies of Marriage Certificate ( showing the whole page )
- Three (3) clear photocopies of husband’s
valid passport ( the data page only ) or Birth Certificate
- Three (3) clear photocopies of
applicant’s passport ( the data page only )
- Completed Report of Marriage Forms (
Available at the Embassy )
- Three (3) passport size photos of
husband to be attached on the report of marriage
- Six (6) passport size photos of
applicant: 3 for passport application form and 3 for report of marriage
- Original and three (3) photocopies of
Death Certificate of late husband or wife and marriage contract if widowed
- Three (3) photocopies of Divorce of
Annulment papers. ( Note: The applicant may have her name changed to that of
her second spouse ONLY if the Petitioner of the Divorce is the foreign
spouse)
B. IF MARRIAGE WAS SOLEMNIZED IN THE
PHILIPPINES

Original and two (2)
photocopies of Marriage Certificate with Registry number

applying for amendment only, fee is £15.00, no pictures needed )

papers. ( Note: The applicant may have her name changed to that of her

REPLACEMENT
OF LOST PASSPORT

Verification Fax will be sent
to DFA , Embassy or Consular Post concerned to confirm the particulars of
the applicant’s lost passport. Fax fee is £3.00 plus £83.00
for replacement of lost passport.

Completed passport

Two (2) copies of Affidavit of
Lost Passport explaining in details how the passport was lost ( forms
available at the Embassy )

Four (4) passport size photos
with white background without eye glasses, taken at least in last six months

Original and one (1)
photocopy of Authenticated Birth Certificate and Marriage Certificate ( if
married ) from National Statistics Office ( NSO ) with official receipt ,and
death certificate if widowed.
- Write the number of the lost passport, place
and date of issue , photo copy of lost passport ( if available )

NOTE :
We do not accept Personal Cheque. Make your payments in CASH , BRITISH
POSTAL ORDER, or BANK DRAFT , payable to Philippine
Embassy.
IF APPLYING BY POST: Enclose a
special delivery or recorded self-addressed envelope for
the return of your passport. The
Embassy in not responsible for any documents lost in the post.
